A.  AUTHORITY:
This notice is given under the Privacy Act of 1974 and the Paperwork Reduction Act of 1995. The Privacy Act and Paperwork Reduction Act requires that the Internal Revenue Service inform businesses and other entities the following when asking for information.  Coast Professional, Inc. is a private collection agency working on behalf of the Internal Revenue Service.

B.  PURPOSE:
The information on the online forms provided by Coast Professional, Inc. and collected on this site, Inc. as a private third party will assist the Internal Revenue Service in carrying out the Internal Revenue laws of the United States. We will comply with Internal Revenue Code (IRC) section 6109 and the regulations hereunder, which generally require the inclusion of an Employer Identification Number (EIN) or account numbers on certain documents filed with the Internal Revenue Service. Information on this form may be used to provide related forms and publications.

C.  ROUTINE USES:
The information solicited on the online forms on this site may be made available to the Internal Revenue Service to assist in making determinations about the level of service provided by Coast Professional, Inc.

D.  CONSEQUENCES OF FAILURE TO PROVIDE INFORMATION:
Respondents should be aware that notwithstanding any other provision of law, no person shall be subject to any penalty for failing to comply with a collection of information if it does not display a valid OMB control number. You are under no obligation to complete any of the online forms provided by Coast Professional, Inc.
